Home and End, Alt with high bit

Moin,

why the keys Home and End are undefined by default. The keys Page Up and
Page Down are assigned. Are they forgotten?

If the terminal send a character with the high bit set for alt+key, how
can I make alt+h opens the opens the help menu and alt+q formats the
paragraph?
